Types of personal information in the PIB

Name, address, telephone number, age, sex, disability, marital status, family status, student number, social insurance number, medical history, psychological history, educational history, financial history, third party opinions about the individual, and individual's own views or opinions.

Purpose for which personal information obtained / compiled and used / disclosed

Personal information is collected by the University because it relates directly to and is necessary for the operation of the Center for Students with Disabilities (CSD). The CSD uses personal information collected about its student clientele to provide them with individualized services and academic accommodations. Personal information is disclosed only for the purpose for which it was obtained or compiled or for a use consistent with that purpose, including the delivery of an integrated student disability program.

Individuals using personal information or to whom it is disclosed

Individuals and university offices to which personal information may be disclosed in the pursuit of the University's delivery of an integrated student disabilities program may include faculty and departmental staff, Student Services, Finance and Ancillary Services, Parking Services and the Human Rights Office.

Individuals included in PIB

Students with psychological, physical or learning disabilities.
