Name, address, telephone number, e-mail address and disability.

Purpose for which personal information obtained / compiled and used / disclosed

Personal information is collected by the University because it relates directly to and is necessary for the operation of the Centre for Students with Disabilities (CSD). The CSD uses personal information collected about its student clientele to arrange for special academic accommodations such as class and exam accommodations. Personal information is also used to secure learning materials in alternate formats such as Braille, taped text and large print. Personal information is disclosed only for the purposes for which it was obtained or compiled.

Individuals using personal information or to whom it is disclosed

Individuals and university offices to which personal information may be disclosed in the pursuit of the University's delivery of an integrated student disabilities program may include faculty and departmental staff and the Library.

Individuals included in PIB

Students with psychological, physical or learning disabilities.
