Name, address, telephone number, age, sex, disability, student number, social insurance number, medical history, educational history, financial history, employment history, and third party opinions about the individual.

Purpose for which personal information obtained / compiled and used / disclosed

Personal information is collected, used, and disclosed in determining admission into a graduate program and then, if admitted, to manage a student's academic career until graduation or withdrawal. Faculty members may refer to student files in response to requests from graduates for letters of reference.

Individuals using personal information or to whom it is disclosed

Graduate program and Co-operative Education assistants and secretaries, staff of the Office of Graduate Studies, academic administrators, and faculty members.
