How to request a service

Please read the instructions below and then click on "New Request" in the right sidebar to get started. We will begin your project when we receive your authorized service request with an account code and signature.

NOTE: This isn't the place to request projectors, room or equipment setups, etc. For those services, contact Audio/Visual Services, which is part of IT Services.

Instructions

Please read these instructions before submitting an online service request for the first time. You can also download these instructions as a PDF file.

  1. Log in to the system with your SFU ID. If you are already logged in to the SFU authentication site, this step won't be necessary.
  2. Click on "Create new" to create a new service request. You can also see a history of previously submitted requests on this page.
  3. Complete the form with all required information. Note that you can select more than one type of service. If you select "Video," you will be prompted to provide details about the event that you would like us to videotape. If you wish to submit relevant documents such as room setup descriptions, previous service requests, logos, or event schedules, check the "Add attachment" box. You will be prompted to add your documents after you "Save" the service request.
  4. Click on "Review and save."
  5. Review the information you are about to submit. If you wish to make changes, click on "Make changes," revise the information, and again click on "Review and save."
  6. When you are satisfied with the information, click on "Save."
  7. If you checked the "Add attachment" box previously, you will be prompted to attach the file(s) now. If not, you will be brought back to the initial screen, where you can view your previous and current service requests.
  8. Once you have submitted your request, you will receive an email with the service request attached, along with further instructions. You will still need to print a paper copy of the service request form and return it to us with an account code and authorizing signature by email (tlcentre@sfu.ca), fax (778-782-4900), or mail before we can begin working on your project.
  9. When you are finished, remember to log out of the system.
