Registration Information
How to Register
By mail
Complete registration
form, print, and mail to SFU Conference Services at the address below.
Please submit separate forms for each person from your institution.
By fax
Use your Visa
or MasterCard and fax your registration form/s to (604) 291-3420.
Methods of Payment
You can pay by cheque using either Canadian or US dollars
(payable to Simon Fraser University) or by VISA or MasterCard.
Sorry, Simon Fraser University does not accept American Express or Diners Club.
Registrations will not be processed without fee payment.
Cancellations and Refunds
Full refunds, less a $25 processing fee, will be granted
to cancellations received by Thursday, May 31, 2001.
No refunds will be issued after that date. Refunds will be mailed after the
conference. Substitutions are permitted.
Receipts
Receipts will be available for collection at the Conference Registration Desk. You will be sent an email as confirmation of your registration.
