ORDERING ACCESS CARDS

Advances in technology have allowed security greater opportunities to not only control access, but also to increase user accountability with the implementation of electronic control devices. At SFU, this technology allows for central administration of multiple academic and service facilities across campus.

To Order an Access Card:

1. An access card will be issued upon receipt of a completed and authorized Card Requisition Form.  Click here for a blank PDF form.

All cardholders, including SFU faculty and staff, must pay a refundable deposit for their card.

A service charge is paid by the department.

2. From the time of receipt, we require up to 3 business days to process the requisition.

3. Cardholders are limited to one active access card.

4. Cards must be assigned to an individual who is personally responsible for its use. They will not be issued for general use by any department.

5. Lost/stolen cards must be reported to Campus Security immediately for deactivation.

Important Notes for Cardholders:

1. Access cards are the property of SFU Campus Security. The access card is on loan to the cardholder and can be confiscated by Campus Security if deemed necessary.

2. Lending or borrowing an access card will result in retraction of access privileges by Campus Security. Cards are for use by the authorized cardholder only.

3. Cards which are not functional due to normal wear and tear will be replaced by Campus Security at no additional cost to the cardholder. Cards which are lost or damaged will be replaced at the cardholders’ expense.
