Minor errors and typos

• p.61, line 25: V₁ should be changed to C₁.
• The entry "Bohme" is alphabetically displaced in the index.
• Problem 5.5.17: The sentence should end up with the question mark.
• Ref. [La87b]: The publication year should be 1987 (not 1978).
• Ref. [LN99]: -- instead of - for page range.
• Abbreviations of journal names (Discr. / Discrete Comput. Geom., Europ. / European J. Combin.)
• Index entry "\Pi-face" should be indented.
• Lemma 7.1.2: A comma before "then" may be added.
• p.122; in the sentence preceding Theorem 4.4.13 change "negative" to "affirmative".

No other errors have been reported so far.
