All credit card transactions will be processed in Canadian dollars and are subject to current exchange rates.
Complete the online form and mail with your cheque made payable to Simon Fraser University to the address provided below. Forms can also be faxed and cheques mailed separately.
Note: Faxed or mailed registrations will not be processed until payment is received.
Full refunds for the conference fee, less a $20 (Students, Seniors, Disabled and Unwaged $10) processing fee will be granted to cancellations received by Wednesday, June 4, 2008. No refunds will be issued after that date. Refunds will be mailed after the conference and will not be given to no-shows. Substitutions are permitted.
If you register online and pay by credit card, you will receive immediate confirmation via our server.
If you pay online by email transaction, please print out your receipt as this will be the original copy. If you pay by cheque, you will be given your receipt at the conference registration desk.
The participant list will show the names of attendees whose registrations are received by Conference Services on or before Wednesday, June 4, 2008. Although every effort is made to ensure accuracy, Conference Services cannot be held responsible for errors and/or omissions. The list will contain your name, organization’s mailing address, and email address.
