Term comes from the use of a film loop in the early days of film post production.  

May be needed if scene was shot on location outdoors in a noisly location , if the director wants to change the performance, if dialogue needs to be changed (why actors say “freak you” on television) or if there is a technical glitch.  Now called ADR.
