I'll point out that voluntary response surveys - such as this one - yield results which really should not be trusted. (Think Google reviews of courses.) Universities have statisticians and economists who could design surveys to gather much more reliable data.

The university is spending a non-zero amount of money on this survey. So it may even be worth completing it with the single line: "The format of this survey is unprofessional; the exercise is a waste of money, and is unworthy of a university." Or: 'You are wasting our time with this voluntary-response survey crap'.
