Create form fields with Acrobat

Choose Tools > Advanced Editing > Forms > Show Forms Toolbar.

On the Forms toolbar, select a tool and do one of the following:

Drag the cross-hair pointer to create a form field in the desired size.

Double-click on the page to create a form field at that location using the default size.

Specify the properties for the form field in the Properties dialog box.

Choose View > Toolbars > Properties Bar to display that toolbar. Then select a form field and edit the field’s properties in the Properties Bar.
