Coalition Member Fee: US $400 / CDN $600
Non-Member Fee: US $450 / CDN $675
Multiple Registrations
Register two people from the same institution at the Early conference
fee and each additional registrant from your institution will pay
a reduced fee: US $300 / CDN $450
FULL Conference Fees (Postmarked or faxed after Monday, September
16)
Coalition Member Fee: US $450 / CDN $675
Non-Member Conference Fee: US $500 / CDN $750
Multiple Registrations
Register two people from the same institution at the full conference
fee and each additional registrant from your institution will pay
a reduced fee: US$ 350 / CDN $525
Notes:
- All presenters are expected to pay the conference registration
fee
- There are no reduced fees for presenters/participants wishing
to attend only part of the conference
What your fee includes
The conference fee includes admission to all sessions, beverage
breaks, meeting materials, Sunday's opening reception, Luncheon
on Sunday and Monday, Breakfast on Monday and Tuesday, and reception
and banquet on Monday evening.
Guests
The guest fee includes the Sunday reception and the Monday banquet.
Guest Fee: US $100 / CDN $150
How to Register
Please submit separate forms for each person from your institution.
If you are registering as an additional person from your institution
at the reduced fee, please submit your form with the other registration
forms.
Online
Complete the online
form. You will be given the option of paying for your registration
fees online via a secure server (VISA or MasterCard).
By mail
Complete the printable registration form(s)
and mail to Simon Fraser University Conference Services at the address
below. The printable registration form is available as a PDF file.
To view this file, you will need to install the Adobe Acrobat software
which is available as a free download from www.adobe.com/acrobat.
By fax
Use your VISA or MasterCard and fax your registration form(s) to
604.291.3420.
Methods of Payment
You can pay by check or bank draft in US or Canadian dollars made
payable to Simon Fraser University or by credit card (MasterCard
or VISA). Sorry, Simon Fraser University does not accept American
Express or Diners Club.
All credit card transactions will be processed in Canadian dollars
and are subject to current exchange rates.
Registrations will not be processed without fee payment.
Cancellations and Refunds
Full refunds, less a US $25 / CDN $37.50 processing fee, will be
granted to cancellations received by Monday, September 23. No refunds
will be issued after that date. Refunds will be mailed after the
conference and will not be given to no-shows. Substitutions are
permitted.
Pre-Registration List Deadline
The Pre-registration List reflects attendees whose registration
was received by Conference Services on or before Monday, September
30, 2002. The list will contain your title, name, mailing address,
phone number and email address.
Receipts
Receipts will be available for collection at the Conference Registration
Desk. You will be sent an email as confirmation of your registration.

To register for the CUMU Conference online, please proceed to the
online registration
form. You will be given the option of paying for your registration
fees online via a secure server with VISA or MasterCard.
You can also print the online form and mail it along with your
check. You will receive a confirmation message that your online
registration has been received and will be processed at the time
of payment.
