If you are not registered go to the Not Registered, complete the registration form and submit your registration. After the Office of Research Ethics (ORE) receives the request to be registered you will receive a response from dore@sfu.ca confirming your registration.

 

After you are registered you may log in using your SFU email address, with the password you selected for registration.

All documents must be in pfd format. The application procedure must include two documents, and the application cannot be submitted until those documents have been uploaded: (a) A detailed description of the study, (b) The consent form(s) that will be used.

 

NOTE:  For the purposes of the ethics application, only ONE principal investigator can be assigned and that principal investigator must be an SFU faculty, staff or student.

 

Other supporting documents may also be uploaded in pfd format.

Help files and a check list are available in the application software.

Important Note: You may save your applications without submitting them. This will allow you to develop applications over a period of time if needed. After the application has been submitted it cannot be changed until it has been reviewed by the Office of Research Ethics.

If students are applying for ethics approval the student's Supervisor will be sent a link that allows the Supervisor access to the application by email and asked to confirm that he or she has approved the application by sending an email to dore@sfu.ca.

Note: For all graduate student applications, the supervisor must be shown as a co-applicant and must be a Faculty Member of Simon Fraser University.

If you are applying for more than one study or experiment, a separate application for each experiment may be created under the same user account. Supervising faculty will also be able to access all applications of projects they supervise.

Applications are approved for the following terms:

Faculty without grants: 4 years or end of appointment term

Faculty with grants: for the term of the grant, or for 4 years, or the end of an appointment term

Graduate Students: 3 years or until no longer a registered student at SFU

Undergraduate Students: 2 years or until no longer a registered student at SFU

Staff: 3 years or the end of appointment at SFU

For minimal risk applications the age of consent is 16 or older. For non-minimal risk applications the age of consent is 19 or older. All students registered at Simon Fraser are considered adults for the purposes of consent.

Postdoctoral fellows whose fellowship payments are scheduled to begin prior to seeking Ethics Approval for studies that require Ethics Approval are asked to declare their understanding of the requirement for Ethics approval using a Declaration of Intent Form (PDF format).

Minimal risk applications are approved by the Director, Office of Research Ethics, on behalf of the Research Ethics Board in accordance with University Policy R20.0,/ http://www.sfu.ca/policies/research/r20-01.htm. This approval allows the research project to begin.

The Research Ethics Board ratifies these approvals at the monthly meeting and at that time may have additional requirements to be implemented.
