Field Activity Plan

Principal investigators must submit annual Field Activity Plans at least 2 weeks ahead of any fieldwork. Field Activity Safety Guidelines and relevant forms are available from the Safety & Risk Services Field Activities webpage.

For off-campus activities within Canada that are categorized as “high or extreme risk” (based on SRS’s Risk Matrix) use the longer form ‘Field activity plan template’ and submit this directly to the Field Safety Advisory Committee (FSAC). They will review and forward a copy to the Chair for approval.

For low/medium risk field activities within Canada complete the (shorter) ‘Low and medium risk field activity form’ and submit this to Melissa Stephens (biscsec@sfu.ca).  Low/medium risk activities do not need a full field activity plan, and the Chair now only approves medium risk undergraduate activity (otherwise responsibility for compliance falls to the PI). Low risk field activities no longer require Chair approval. The department will keep the forms and any accompanying emergency contact information on file.
