Forms

The following provides a selection of forms for various aspects of graduate studies at Simon Fraser University. As all forms are in Adobe's Portable Document Format (PDF), you will need the free Adobe Acrobat Reader application installed on your computer in order to view them.

 

These forms can be printed and filled out manually or completed directly on your computer by typing the necessary information, then printing. However, since duplicates and signatures are required to process them, the forms cannot be submitted online at this time. Applicants must print out the required number of copies and mail them to the appropriate address. See specific forms for instructions. Please note that when using Acrobat Reader, use the "Zoom-In" Tool (magnifying glass icon with the + sign) to enlarge the form and have a better view of your form input.
