This form is for people who have graduated from SFU and are alumni.

Did you know that an active SFU ID is needed to:

• Order transcripts or apply for graduate studies through go.sfu.ca
• View your SFU payroll history at myinfo.sfu.ca

If you need to reactivate your SFU ID, please fill out the form below. You will be required to include your SFU student number. Can't remember it? Please request it by going here. You should receive your student number within 3-5 business days.
