Make a report

If you are an SFU student or employee, you can submit a report about bullying and harassment that you have experienced by another SFU student or employee or that has taken place on SFU property. There are two options for submitting your report:

  • Meet with a case manager who can answer your questions about the reporting process and assist you with submitting the report.
  • Submit your report directly through the online reporting form. 

If you are a unionized employee, you are encouraged to contact your union representative before submitting a report.

Once a Report is submitted to the University, it is directed to the Responsible Office for action. The identity of the Respondent (the person who the complaint is about) determines the Responsible Office:

Information on Anonymous Reports

Please note that when filing an anonymous report, you will need to provide your contact information in order to inform you of the Responsible Office’s decision with respect to jurisdiction and in case additional information and details are needed for that decision. Otherwise, SFU may be unable to proceed with an investigation based on an anonymous report due to lack of evidence.

Consent for SFU Responsible Office to action the complaint

By checking this box, I agree and understand that:

a)      I’m submitting a Bullying & Harassment Report to SFU under the Bullying & Harassment Policy;

b)      This report will be used for the purposes of an internal review to determine whether SFU has the jurisdiction to investigation under the Bullying & Harassment Policy;

c)      If the Responsible Office/SFU begins an investigation, then a copy of this report will be provided to the Respondent; and

d)      Failure to give my authorization to SFU will prevent the university from taking follow-up steps in response to my Report, including initiating an investigation.

The information I provide to Simon Fraser University (“SFU”) on the Bullying and Harassment Report Form (“Report”) is collected under the authority of the University Act  (R.S.B.C. 1996, c. 468), the Freedom of Information and Protection of Privacy Act  (R.S.B.C. 1996, c. 165) (“FIPPA”), the Workers Compensation Act  (R.S. 2019, c. 492), the Occupational Health and Safety Regulation (B.C. Reg. 296/97), SFU’s Bullying and Harassment Policy and its Health and Safety Policy (GP-17).
