Disabilities

Chronic

This category encompasses many different physical disabilities and there are a tremendous range and variability in these disabilities from one person to another. However, all chronic health disabilities are medical conditions that significantly limit a student's ability to function in an educational environment. Some people falling under this category may require a wheelchair, whereas others are able to move freely but have impaired hand or motor functions which make writing difficult. Medical conditions that fall under this category of disability may include, but are not limited to:

  • Chronic pain
  • Chronic fatigue (CF)
  • Arthritis
  • Cancer
  • Diabetes
  • Kidney disease
  • Hepatitis
  • Allergies
  • Chemical sensitivities
  • Asthma
  • Cardiovascular problems
  • HIV/AIDS
  • Fibromyalgia
  • Lupus (SLE)

Documentation Requirements

All students seeking to register with the CAL must submit the Application for Services Form as well as supporting documentation, which must be filled out according to the guidelines below.

Appropriate professional: physician

Documentation should include:

  1. A clear statement of the diagnosis and a summary of present symptoms
  2. A statement indicating whether the disability is permanent/ongoing or if the disability needs to be reassessed periodically (specify time period if possible)
  3. Medical information relating to the effect of the disability on the student’s ability to meet the demands of a university environment (including the impact of medication or other treatment on performance)
