February 29th is the deadline to make RRSP contributions that are to be included on the tax receipt for the first 60 days of 2012. Sun Life must receive these contributions no later than 2 P.M. (ET) on February 29th. Please send your cheque, payable to Sun Life, to Sun Life Financial, Group Retirement Service - PO Box 2025 Stn Waterloo, Waterloo, ON N2J 0B4 along with this form. The Client ID is C0H19, and the Plan number is 04.
