CAS Services Application Form

Any application at SFU can use CAS to authenticate users. See CAS for web applications for information on setting up CAS. To use CAS, applications must be registered as a service within CAS.  Fill out the form below with the required information so that your service can be registered in CAS.

The name of the contact for the service being set up
Email address of the application contact.
This is the name that will appear on the CAS page when users are asked to log in to the service.
This is the URL of service that will be passed in to identify which service is calling CAS. Use regular expressions if setting up a service that will have multiple URLs.
Please provide a brief description of the purpose of this service.
Check this box if you don't want real usernames returned by CAS, but instead unique, opaque identifiers.
