Incident reporting

All health and safety incidents, including near misses, must be reported to the involved person’s supervisor and online though the SFU Health & Safety Incident Reporting System within 24 hours of the incident occurring. If the involved or injured person is unable to complete the report, it may be filled out by the person's supervisor or another individual with knowledge of the incident.

*If you chose to contact an ISOS Assistance Center to report an emergency, any personal information you provide will be stored outside of Canada.

International SOS is under contract with Simon Fraser University to provide medical, security, and logistical information, evacuation and repatriation, and emergency travel and accommodation arrangements. The information on this form is collected for the University under the general authority of the Freedom of Information and Protection of Privacy Act (R.S.B.C. 1996, c165). It is related directly to and needed by International SOS and the University to provide incident response and emergency evacuation assistance.

The information will be used to administer your account, alert you of advisories pre and during trip, locate and assist during emergencies, and contact emergency contacts if required.
