All health and safety incidents, including near misses, must be reported to the involved person’s supervisor and online though the SFU Health & Safety Incident Reporting System within 24 hours of the incident occurring. If the involved or injured person is unable to complete the report, it may be filled out by the person's supervisor or another individual with knowledge of the incident.
*If you chose to contact an ISOS Assistance Center to report an emergency, any personal information you provide will be stored outside of Canada.
The information you provide through the ISOS Assistance Center is collected under the authority of the Freedom of Information and Protection of Privacy Act (R.S.B.C. 1996, c165) S. 26(c) and the University Act (R.S.B.C. 1996, c 468). It is related directly to and needed by the University to administer the Simon Fraser University Travel Safety Program. The information can be used to provide assistance in the event of an emergency while you are abroad on a university-related activity.
