Claims & Incidents

Visit the Risk Management website to file a claim or alert us to damage or theft of university property, including SFU vehicles. 

Risk Management should also be immediately advised of any reparation demands or notice of legal action against SFU.   

Health & safety incidents

Report all campus health & safety incidents including "near-misses".  An incident is an event or occurrence including accidents, injuries, occupational illness and hazardous material spill or release.  A near-miss is an event or occurence where there was the potential for accident, injury, occupational illness and hazardous material spill or release.

**Note: Effective August 30, 2021, Environmental Health & Safety has implemented a new Online Incident Reporting system.  For employees, you will be walked through registering for an account in the system the first time you use it.  Subsequently, you will login to the system using the email and password that you registered, and have access to your past reports.  For students and visitors, only the format of the report has changed.  For more information, visit the incident reporting FAQ page.

Examples include:

  • Injury
  • Occupational illness
  • Fire or explosion
  • Hazardous material spill or release
  • Near-miss incident (where the potential existed for any of the above)
